Self referrals

Most referrals to other specialists will require you seeing or speaking with a GP. However, there are a number of local services, listed below, that you are able to make self referrals to.

Youth Counselling

Relate has local Centres across the UK. The services each centre provides vary because each centre tailors their services – such as couples counselling, sex therapy and counselling for children and young people – to the needs of their local community.

Wiltshire Sexual Health Service (WiSe) is a free, non judgemental and confidential NHS service providing STI testing and treatment, contraception and HIV services in Wiltshire.

Podiatry

The Podiatry Service provides a comprehensive range of specialist and general clinical interventions. This includes health education and promotion and the effective triage, assessment, diagnosis and treatment of foot problems.

Young Carers

A young carer is someone aged 18 or under whose life is affected by caring for at least one family member, over and above just ‘helping out’. Young carers might look after: Parents, grandparents, siblings, close relatives.

Physiotherapy

Do you know that you can self refer to the physiotherapist for any musculoskeletal issue without seeing a GP. If you would like to self refer to the physio please complete the forms below for either Salisbury or Andover.

Andover – Following submission of your form, please allow  five working days before contacting to arrange your appointment.You can contact our central booking office by calling 0300 373 0212 or physioenquiries@southernhealth.nhs.uk to book your appointment.

Audiology

Audiology at Hampshire Hospital NHS Foundation Trust offers self-referral for patients aged 18 and over who have gradual onset hearing difficulties/loss or bothersome tinnitus (without vertigo or ear pain). Please contact the service directly to self-refer.

Provide as much information as you can as this will help us to process your request.

If your referral was not requested by the surgery please contact the provider directly.

Tracking a referral

To track an existing referral:

NHS e-referral (choose and book)

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.
